Confirmation Registration 2020-2021

Online Registration is the only registration for Confirmation at First Lutheran. You will need your insurance card, phone numbers, emails and your youth to help fill out how they would like to be involved with Confirmation Ministry! The form cannot be "saved", plan for 13-20 mins to complete the form!

Below you will find the simple online form to fill out to register your child for the Confirmation Ministry. Follow the steps below and you'll receive a response from First Lutheran that you have been registered! If you have any questions, feel free to contact Michael Beckmann in the church office via email: michaelb@firstlutherancr.org

Steps to Regisiter

1) Fill out form (online) click submit at the end
2) Turn in $25 check made out to First Lutheran Church
3) Check your email for a response within 5 days
4) Stay tuned for more details regarding the fall schedule!

    Parents and legal guardians of minor children are asked to complete this form. The information requested is designed to assist the church in providing for the safety of minors during church-sponsored activities.

    Please provide an email to add to our weekly distribution list for Confirmation Ministry. This is our main way of communicating weekly details to our families involved with the Confirmation Ministry throughout the year.
    If you would like to have more than one email associated with the communications from the Middle School Ministry, please add it here!

    Consent and Certification

    Consent for Participation
    ​I, the undersigned, being the parent or legal guardian of the child named above (the “child”), do hereby consent to the participation of my child in all of the regularly-scheduled activities of all programs for youth of First Lutheran Church, Cedar Rapids, Iowa, (“Church”) for the period September 1, 2020 through September 1, 2021, including field trips, campouts, swimming, boating, hiking, sporting events, and any other activities customarily associated with church youth programs. 

    Consent of Photos or Videos
    ​I, the undersigned, being the parent or legal guardian of the child named above (the "child"), do hearby give permission to First Lutheran Church of Cedar Rapids, IA to take pictures and/or video of my youth during Confirmation. These pictures or videos may be used for promotional purposes which may include posting on First Lutheran’s website, Facebook page, or Twitter. Please note: Youth will not be identified by name, and no photos will be posted of youth in vulnerable poses.

    Medical Questionnaire


    Medical Treatment Authorization

    I understand that reasonable attempts to notify me will be made in the case of a medical emergency involving my child. However, in the event that I cannot be reached, I authorize the calling of a doctor and the providing of necessary medical services in the event my child is injured or becomes ill. 
    I authorize any one or more of the following persons: Pastor Steve Knudson, Pastor Craig Brown, Michael Beckmann, or if none of them is available, then any adult supervisor, to make emergency medical decisions on behalf of my child if such decisions are required by law or deemed necessary or advisable by a health care provider. I understand that the Church and the persons named above will not be responsible for medical expenses incurred solely on the basis of this authorization.

    I agree to notify the Church in the event of any health changes which would restrict my child’s participation in any normal youth or children’s activities. I also understand that the adult supervisors reserve the right to restrict my child from any activity that they do not feel is within the physical capabilities of my child.

    A copy/online version of this form shall be deemed to be as effective as the original.

    Registration Form- Ministry & Involvement

    Worship
    Centering our lives on God is the foundation to our faith life.  Please choose the worship service you are committing to make your primary worship time.  It is understood that you will attend worship at other times (due to personal schedule, participation in choirs, etc.) or even in other congregations.  We ask that you choose one as your primary service because the very act of making this commitment is an encouragement to be faithful in attending.
    Worship Participation
    ​We ask that you participate in worship 4 times during the ministry year. (All youth will be asked to Acolyte at least 2 times during the year.)  Check the boxes you are interested in. We will provide training and adult supervision of all roles.  

    Christian Camp Experience
    Connecting with each other and with God in a fun but meaningful way is what happens at a Christian Camp.  Because we recognize the value of such an experience we ask that you commit to attending either a weekend Dirty Feet Retreat or a week at a Christian Camp* sometime between the summer before your 6th grade year and the Fall of your 9th Grade year.  Please mark what (if any) you are willing to commit to this year.*By this we mean denominational camps like EWALU, Riverside Lutheran Bible Camp, and Camp IO-Dis-E-Ca.  These camps focus on bible study, include worship, and are built around faith growth and development.  While wonderful camps providing a great experience, Camp Wapsie, FCA Camps, and other nominally Christian camps do not have the same focus.​
    Mission Trip Experience
    As a Confirmation Ministry we believe it is important to provide our confirmation youth the opportunity to serve others. This year we are including the service nights into our community nights. We do encourage families to make a point to come to the service nights each month as a way to build up the importance of serving others, and to look for ways to serve outside of the church!

    Parent/Guardian Leadership & Support
    As parent/guardian(s) you play the vital role in the faith development and growth of your child.  Statistical evidence proves that parents/guardians are the number one faith influence on their children.  We are committed to doing all we can to support you.  The more your children see you engaging in these ministries, the more likely your children will engage in their young adulthood.  

    We ask you to make the following commitments:

    1) To attend worship as a family.  Worship notes are meant to create space for you and your youth to connect the message in worship to your everyday life. You can usher or greet as a family.  Your child can read scripture. You can acolyte together!

    2)  To grow your own faith.  Become a faith mentor, seek spiritual direction with Marcie Watson, join in one of the many opportunities during the Sunday Morning Education Hour, Worship TOGETHER, or serve dinner to people in need on Saturday nights at church.  These actions model for your children the continued growth that accompanies a living faith.  

    3)  To talk with your children about what they are learning and participate on parent nights.    You are always welcome to attend on Wednesday nights and we will work to communicate with you often. Use the Confirmation Family Blog as a way to lead devotions after Wednesday nights. Use the Faith 5 to lead a simple devotion. Bless your children!

    4)  To pray with and for your family.  Pray at meals and before bed.  Stop to pray in the middle of a tough time!  Pray for our pastors and confirmation leaders.  Time spent together in prayer, reading scripture, reading a devotional book, or discussing the week’s sermon can be a great way to grow your relationship with your family and with God.
